The diabetes team provide diagnosis and ongoing management for patients experiencing all types of diabetes. 

Patients have access to a range of specialists and clinicians including an endocrinologist, endocrine registrar and diabetes educators. The team work alongside other specialists and programs to offer the best care for you. 

What do we do?

The diabetes team provide care for all types of diabetes for diagnosis and ongoing management.

Specialist diabetes clinics include:

  • Paediatric Diabetes
  • Adult Diabetes
  • Diabetes in pregnancy
  • Young Adult Diabetes
  • Nurse-led clinics

Specialist services include:

  • Continuous glucose monitoring
  • Insulin pump initiation for paediatrics and adults
  • Bolus advisor meter service
  • Ambulatory insulin stabilisation
  • New diagnosis Type 1 diabetes, adults and paediatrics
  • New diagnosis Type 2 diabetes adults and paediatrics
  • Gestational Diabetes

The Diabetes Centre at Grampians Health Ballarat is a member of the National Association of Diabetes Centres (NADC) and an accredited Tertiary Diabetes Service. We are a key provider of Diabetes education to health professionals.

Who do we care for?

All patients referred for diagnosis and management of diabetes.

How can you access this service?

A referral from your General Practitioner or other health professional.

Each referral will be assessed based on greatest need and urgency for appointment times.

A confirmation letter will be sent with the details of what you need to bring to your appointment.
