Heart Failure Symptom & Early Review Helpline
A specialist Heart Failure Nurse Practitioner is available to support patients after they leave hospital.
Patients (or their support person) can call and get advice/support from an experienced Heart Failure Nurse Practitioner if they have concerns about the signs and symptoms that they are experiencing.
If you are experiencing chest pain or a medical emergency, please call the ambulance on 000.

When should I call the Heart Failure Help Line?

  • Worsening shortness of breath
  • Increased swelling in your ankles or legs
  • Unable to breath when lying flat
  • Your weight has increased by 2kg or more over the past 2 days
  • Increased dizziness, especially when you stand up

If the Heart Failure Nurse Practitioner is with another patient, please leave a message. You will be usually be called back within two hours.

All non-urgent requests for information/support may be sent via email to heartfailure@bhs.org.au 
These emails will usually be replied to within 48 hours.

How can the Heart Failure Nurse Practitioner help me?

When you call, the Heart Failure Nurse Practitioner will ask you questions about what symptoms you are experiencing as well some general questions.

The nurse will then offer one or more of the following

  • Advice/support over the phone
  • Book you in to the Heart Failure Clinic for early review
  • Link you back in with BHS at Home
  • Request that you arrange to see your GP
  • Advise you to attend the Emergency Department (ED)

If you are advised to see your GP or report to the Emergency Department (ED), the Heart Failure Nurse Practitioner will notify them prior to your arrival.
