The Community Rehabilitation program provides a range of rehabilitation services for adults who need access to Occupational Therapy, Physiotherapy, Speech Pathology, Social Work, Exercise Physiology and Allied Health Assistant.

What do we do?

Community Rehabilitation provides patients with assistant to manage their condition and work towards recovery goals after a neurological condition or medical diagnosis.

This intervention is client centred and orientated towards each individual’s goals and needs.

What can you expect?

The program is delivered primarily at the Grampians Health Ballarat Queen Elizabeth site. Clients may be treated on an individual basis (centre, community or home based) or in small groups.

How can you access this service?

A referral from a medical practitioner, specialist, clinician or client can be submitted to access this program.
