Planned Activity Groups (PAGs)

Service Description/Overview

To engage people in meaningful group social activities which draw on personal strengths, build capacity, encourage independence and involve them in their community.  The positive, supported environment provides social and respite opportunities for participants and their carers to prolong independent community living.

Our three PAGs were formerly known as Adult Day Centres.The programs are facilitated by staff with a minimum qualification of Certificate III Aged Care. Staff may provide limited assistance to individuals with general activities of daily living, such as eating, personal hygiene and mobility. Participants do need a minimal level of independence with mobility, and need to be able to cope within a group setting where the choice of activities may involve short excursions. The progress and care of all participants within our programs is planned and monitored with family, carers and supporting teams.

This Program is supported by financial assistance from the Commonwealth and Victorian Governments.

It is not registered with NDIS but may provide places for participants meeting the attendance criteria at full cost recovery fee.

Area Serviced

All Planned Activity Groups are located in the Ballarat region and service the Greater Ballarat Area.

Fees

Participants are charged a nominal fee to attend PAGs.This fee assists with daily programming costs, morning and afternoon tea, the activity program and a two-course lunch.

Alternative activities at minimal additional cost are available at times.  Participants ineligible for CHSP (Commonwealth Home Support Program) or HACC (Home and Community Care) funded programs will be charged at full cost recovery for service.

Referrals

Enquiries and referrals are welcome from the general public and health professionals.

Referrals to this program are to be made using the My Aged Care Portal (MAC) for over 65 aged CHSP eligible clients.
